How AVONDALECares Works
Step 1
Your prescription will be sent to Lombard Pharmacy and will help SAVE YOU MONEY. Lombard Pharmacy will verify your insurance coverage.
Step 2
You will receive a text message from Lombard Pharmacy with a link to view your prescription within moments of receiving the prescription from your healthcare provider. You can also use the phone number below to speak to a pharmacist if you prefer.
Step 3
AVONDALECares*:

- Commercially insured patients with approved prescription will pay no more than $75 and could be as low as $0 depending on insurance coverage
- Patients may have to pay $99 if prescription is not approved
- If applicable, Lombard can assist you with a Prior Authorization
- Prescription is shipped for free within 2-4 business days, pending completed prior Authorization if needed

Important information for healthcare providers: When you send your patient’s prescription to Lombard Pharmacy, Lombard Pharmacy will check the prescription to make sure it is complete, as well as check the patient’s prescription benefits.
While your patient’s prescription does contain protected health information, all parties comply with the Health Insurance Portability and Accountability Act. If your patient does not wish to have their prescription sent to Lombard Pharmacy, please send your patient’s prescription to a pharmacy as directed by your patient. By sending your patient’s prescription to Lombard Pharmacy, you acknowledge that your patient has consented to participate in the AvondaleCares program.
Terms and Conditions: Offer cannot be combined with any other rebate or coupon, free trial, or similar offer for the specified prescription. Not valid for prescriptions reimbursed in whole or in part by Medicaid, Medicare, VA, DOD, TriCare, or other federal or state programs (including state prescription drug programs).
